Our solution is already certified by the IT authority of the Portuguese Ministry of Health and the National Authority of Medicines and Health Products, following international standards and is fully aligned with European Digital Health initiatives. Our platform is designed to be fully integrated into any clinical system, allowing automatic contextualization in the patient and physician's process, intelligence in search engines, automatic suggestion of doses, quantities and durations, use of prescription protocols, quick access to history and favorites list allow to achieve a simple and fast prescribing process. In the near future, this platform will be used by any registered physician through a Software as a service model. Our value proposition: Poor adherence to appropriate medication therapy has been shown to result in complications, death, and increased health care costs. Medication adherence in patients with chronic conditions such as diabetes, hypertension, hyperlipidemia, asthma, depression and cancer is a significant problem requiring intervention. Taking advantage of an ongoing project at the Champalimaud Foundation, we developed an Intelligent prescription system fully integrated with the clinical system and with a patient/physician App that is fast, easy to use, promoting patient adherence and engagement with the therapeutic plan. The system will enable effective reconciliation of outpatient medications, by support, motivate and providing full transparency, for patients and clinical team, at every step on their treatment journey, while reducing waste and cost by managing medication stock at the patient's home, avoiding over-prescribing, measuring medication adherence, tracking patient medication and refill requests. Our additional goal is to close the gap between clinical systems and the world outside the hospital regarding all information related to the prescription and administration of medications. Poor compliance and high levels of polymedication can lead to errors and severely compromise treatment success, especially in chronically ill and cancer patients.
